Receive a call.
Car accident at Federal Highway, near Guiness Anchor Beer factory.

Involving a few casualties. Ambulance dispatch and arrive within a few minutes (just nearby from the hosp)

bomba arrived 5 minutes earlier(as usual) - their job to pulled out victim from wreckage.

2pm, Emergency Dept. UMMC
Receive a phone call from National Emergency Hotline Service 999 - A trailer from Port Klang had accident and the driver stuck inside the wreckage.

10 minutes later, the ambulance team arrived. support team, Bomba & Penyelamat arrived 5 minutes earlier to carry out the patient. meanwhile, I sneak out and give O2 and IV drip to replace the fluid and prevent 'hypovolumic shock'. (sebenarnya x boleh masuk site, medical team should wait outside of the danger area and wait til the rescue team carry em' out) - the incident area looks safe for me to give 1st responder aid since the blood from forearm pouring out.

1.30hour later, Bomba & JPA manage to carry him out of the wreckage.
my full management at side.
On board, emergency dept inform departure from scene.
Patient fully conscious and looks in deep depress. Of course he knows what is going to happend with the hand.

patient sustained a crush injury at right hand and vascular problem.

send to ER, UMMC stabilize and send directly to operation theater (OT) for further management.

Poor Him.

Aku hampir2 menyaksikan kekuasaan Allah yg maha esa.
tat kala fikiranku masih di Cherating disulami dgn kenangan2 indah disana..

Setelah penat memandu 5jam terus di siang hari. Malamnya pula, giliranku bekerja.

seorang pesakit 70tahun, lelaki, melayu yang datang pada 5pagi
ditemani oleh ahli keluarganya. mengadu 'chest pain','upper limb numbness', nausea + vomiting, sakit perut nak buang air besar
(typical sign+symptom of acute miocardial Infarct - serangan jantung yg boleh membawa maut)

tanda vital tekanan darah didapati rendah. manakala bacaan ECG menunjukkan (ST elevation) -serangan jantung.

ubat-ubatan seperti Aspirin 300mg, GTN diberi untuk mengurangkan kesakitan dada.disertakan dgn Iv Morphine + iv maxalon diberi utk mengurangakn kesakitan dan menggalakan pesakit berehat + melawan kesan ubat morphine.

Sepatutnya pesakit diberikan ubat 'streptokinase' (ubat pemecah segala yg tersumbat di dalam saluran darah) untuk mengurangkan kesakitan + pelancaran darah.. malangnya tekanan darah pesakit terlalu rendah dan mengundang risiko besar sekiranya prosedur diteruskan..

ubat2 seperti dobutamine, dopamine, iv gelafundin diberi untuk meningkatkan semula tekanan darah tapi, malangnya tiada sebarang perubahan..

suasana terlalu sunyi ketika itu. cuma kedengaran pesakit mengucap 2 kalimah sya'adah perlahan2 (atas nasihat jururawat) sambil menahan kesakitan..

aku terlalu sedih melihat situasi itu dengan tidak dapat berbuat ape untuk melawan kuasa Allah.pendapat aku, pesakit ini akan meninggal dunia secara perlahan2 dan pasti akan merasai kesakitan bila nyawa dicabut..

tempat yg biasanya tidak boleh ahli keluarga masuk lebih2 lagi ketika aku membuat rawatan, dibenarkan masuk.

Ahli keluarga diberitahu yang pesakit sekarang terlalu tenat dan tapi tidak pasti bila dia akan pergi..
(aku tak pernah anggar hidup/mati seseorang, itu semua kuasa Allah)
aku nasihatkan kepada anak2+saudara supaya meminta maaf serta mengajar 'mengucap' kepada pesakit.

(dipendekkan cerita)
waktu untuk pulang, pesakit semakin tenat..
aku pun pulang kerumah..

malam berikutnya sewaktu bekerja, ku periksa buku catatan pesakit, 5minit selepas ku tinggalkan tempat kerja,
pesakit meninggal dunia

aku tidak pernah menghadapi situasi pesakit yg meninggal dunia secara perlahan2 begitu.. selalunya meninggal mengejut/terus

terlalu sedih, tapi ku simpan di dalam hati utk menjaga tingkahlaku profesional. tetapi didalam hati, sekuat2 mana manusia
tidak mungkin akan menahan ujian + kuasa Allah dan pastinya akan bersedih..

But it went the distance, and in the end Drogba's stupidity proved decisive as it meant Terry got involved in the shoot-out. As much as you feel sorry for Terry, you have to wonder what he was doing taking a penalty. The only centre-half I have ever known that was any good at them is Steve Bruce.

It may be that the Chelsea skipper was after a little bit of personal glory or felt that he had a responsibility to take one, but a true leader also knows when to step back and let his team-mates have their turn.

It was reminiscent of David Batty in 1998 or Gareth Southgate at Euro '96. It's all very well having the bottle to step up, but if you aren't good enough technically then it's not enough.

Trauma dan Kecemasan, University Malaya - 22 January 2007, Jam 0450

AKU SAMBUT BABY LAHIR DALAM TAXI!
Setelah 3 tahun x buat benda tu..but its always fresh on my mind..

Sebuah Taxi datang dengan Melulu laju dengan lampu kecemasan.
Kebetulan aku ditugaskan di kaunter kecemasan..
Pemandu tu keluar dari taxi dan menjerit2 untuk meminta tolong (sebijik macam ayam nk bertelur) hehhe..
aku ckp dkt dia.. "u just relax, ur scream will not help me.. and will make situation worse"

bukak pintu taxi, Si ibu sedang baring disisi suaminya.. dengan kepala anak dah terkeluar.. (ganas tak?)
Indonesian couple..

aku segera mengarahkan partner aku minta bantuan dari org dalam..
bantuan lambat sampai..

aku dapat merasakan baby tu akan lemas kalau x di tarik keluar.. (nak lagi ganas??)
aku bukak panty ibu.. terus tarik dan sambut baby tu.. keluarla kesemuanya dan air2/darah smua..

baby yang dari tak bernafas, biru.. terus menangis dan bertukar menjadi merah kerana darah mula mengalir ke seluruh anggota.. (satu kebesaran Allah yg x semua orang dapat tengok...)

20 saat kemudian.. bantuan sampai bersama 'set sterile'..
tali pusat dipotong.. dan baby dan ibu selamat dikeluarkan dari taxi dan di bawa masuk utk rawatan susulan
(disaksikan oleh orang ramai, free show...) selamba aje depa tengok pompuan tu punya.. ishk2..

Orang ramai ucapkan tahniah kepada aku.. tapi ia lebih wajar diberikan kepada pasangan ibu bapa itu..
aku sangat2 bersyukur kerana baby dan ibu selamat melahirkan dan bertambahlah lagi zuriat orang islam.

so, sape lepas ni nk aku sambut kelahiran dia? hehhe..

sending ill patient tru air plane will be the sweetest thing on my mind!!
a good friend of mine, Dr. Farhat (anaest. Specialist) offered me to join him to transfer
one ill patient who had fall from height has been in hospital for the past 3 months

not very good condition eh? hmm..

Day 1:
12pm reach KLIA, and directly to cargo. (masuk ikut kargo.. trus sampai pesawat!)
since involving the ambassador + agency. we have to meet em' 1st. and the best experience,
naik Garuda Indonesia =) *cewek2 pramugarinya.. aduhh.... ;) x abis2 dgn ada 'apa2 yg boleh
dibantu pak Dokter'? comel x??? hehhehe..
flight around 2.30pm

Smpai2, trus masuk.. tengok mcm mana dia pasang stretcher utk patient nk baring.
slalu, naik flight dah kemas2 + cantik2 aite? ni.. depa tgk dok kemas2 lagi.. isi makanan, etc
pramugari pun tgh kemas2 dapo ;) semementara tunggu dia pasang stretcher, mcm2 aku mintak
dkt pramugari dia.. es' jeruk (fresh orange) etc.. hehhe.. 1 hr 45mins sampai Sokarno Hatta
(jakarta airport) 1 hour back from KL. reach around 4pm (local time)

then, we transit to another flight via ambulance straight Semarang to which in central java,
40minutes on board. (alhamdullillah patient was well and x buat perangai) around 6pm
(gelap dia, mcm dah 8pm KL!!)

As we arrived, 1 ambulance waiting for the patient- to transfer the patient to Home.
Tired to da max! directly to our hotel (all provided by the agency) best x? duit 1 sen pun
x pakai!! dgn VVip Treatment.. mmg lovely ;)

Day 2:
bermula la aku belanja duit 1 juta Dpt cenderamata from the agency and flight back to Jakarta
kalau di ikutkan, plan agency nk hantar terus balik KL.. but we, decided to spend a night at jakarta
Farhat bnyk kwn dkt sini.. so senang sket la.. tido dkt La' meriddien (spelling check pls)
aku betul2 ngantuk dah ni.. Security hotel dia strict betul!! ye la indon kan penah kena bomb!

keluar shopping!! farhat gila local food and reebok! abis dia blasah duit shopping..
i pulak sebok nk beli local stuff! blasah sampai nak abis duit!! TAPI, 1 BENDA JE AKU BENCI DKT SINI
TRAFFIC JAM DIA ADUHHH... 10 KALI GANDA DARI KL!!! xda LRT,KOmuter,Star, etc!! cuma bus + taxi!!
sampai 50 ribu = rm20 aku habis dkt taxi!! jalan2 dkt historical place dia like Monas (monument nasional)
and more.. naik 'Bajet' = tuk tuk (moto roda 3 mcm dkt thai tuu..) murah dan best!! celah2..
xdpt bayang kalau rempit malaysia bawak!! hehhehe..

as usual, 6pm dah start gelap! (Boring gila) smua kedai pun dh nk tutup! KL at least 9pm aite?
hmm...dgn penat2 + shopping bag yg besar tu.. terus belajalan2 lagi.. smpi lewat mlm.

Day 3:
bangun seawal 6.30am (terang mcm 8am) hehhe.. breakfast, check out and cab to airport.
flight 11am tapi 7am dh kluar hotel.. x dpt bayang kalau kuar kul 9pm!! mesti sangkut dkt traffic jam!
hehhe... nangis la kawin dgn orang indon!! ;'(

beli sikit2 dkt tax free shop, jakarta.. 30 minutes sbl flight bru smpi (amaran, dkt indon aje bleh wat mcm tu)
hehhe..
Flight back to KL and reach KLIA around 2pm.
bag besar2 dgn medical emergency equipment, my cenderamata etc.. hehhe..

harap2 ada la lagi peluang part time mcm ni.. anyway, thanx to Garuda Indonesia, the ill patient's agency and embassy of indonesia

My 1st time teaching as a trainning lecturer.

Becoming a good lecturer in clinical posting would be the best moment alive for me.

who knows starting from dream and now its become reality. i've been choosen to join the FRC team 'first responder course' and become a trianning lecturer. i'm so delighted to start teaching but you know, there is always a first experience.

mouth ulcer, keep on repeating the same things, remind em', funny, etc..

hope they will pass the clinical and objective question this saturday. watch out malaysia, my student will born and start to serve the public ;)

now i know how hard to become a teacher/tutor/lecturer. kesabaran pasti teruji.. I always keep it to my self, this is just the beginning.. thanx to Allah for the chance and oppurtunity another milestone in my life hope to further my carrier near future..
